Payment Methods Accepted:

  •  Cash, check, all major debit/credit cards (Visa, Mastercard, Discover, American Express) via Electronic Payment Agreement through Simple Practice 
  • Any declined checks or debit/credit cards may incur additional fees (see Mandatory Disclosure Statement for more information)

  •  Kyle Bridgman, LMFT: Self-pay rate – $150 per session  /  Some insurance may be accepted, speak with therapist for more information 

  • Kevin Taylor, LPCC: Self-pay rate – $100 per session  /  Insurances – Aetna, Cigna and Cigna EAP, Colorado Access (CHP+), Medicaid/CCHA, RMHP

  • Ellie Colpitts, LPCC: Self-pay rate – $100 per session  /  Insurances – Aetna, Cigna and Cigna EAP, Colorado Access (CHP+), Medicaid/CCHA, RMHP

  • Michelle Hernandez, LPCC: Self-pay rate – $100 per session  /  Insurances – Aetna, Cigna and Cigna EAP, Colorado Access (CHP+), Medicaid/CCHA, RMHP

  • Julia Ralston, LPCC: Self-pay rate – $100 per session  /  Insurances – Aetna, Cigna and Cigna EAP, Colorado Access (CHP+), Medicaid/CCHA, RMHP

* Clients will be responsible for any co-pays incurred utilizing insurance at the time of session. Please be aware that co-pay amounts can vary depending on type of insurance used, yearly deductibles, and other factors, and are set by the insurance company, not Project Ember Initiative. 
** Other rate options may be available on a case-by-case basis. Please speak with your therapist for additional options

Cancellation Policy:

  •  Full session charge for appointments canceled less than 24 hours before appointment start time or failure to show for the appointment
  • “Grace” will be given for first late cancel or no-call/no-show and no fee will be charged
  • No charge for cancellations due to sickness or unsafe weather/road conditions. Notify your therapist ASAP if you will be unable to make your session so others on the cancellation list may be scheduled

Good Faith Estimate (valid thru 12/31/23):

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost

  • Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance, an estimate of the bill for medical items and services
  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This included related costs like medical tests, prescription drugs, equipment, and hospital fees
  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider and any other provider you choose for a Good Faith Estimate before you schedule an item or service
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill
  • Make sure to save a copy or picture of your Good Faith Estimate
